On behalf of the Board of Regents and the administration of the University of Guam, I welcome you back to our campus for the Fanuchånan 2021 semester. We are excited for this new academic year because we are returning to in-person classes.
As our University prepares for in-person learning and the full re-opening of the campus, we remain steadfastly committed to the health and safety of our students and employees. Through President’s Directive 2021-02, I have required that all UOG employees be fully vaccinated or have an approved accommodation by Sept. 24, 2021.
Below are requirements for all students for Fanuchånan 2021:

NOTE: These requirements will be updated should the Governor or DPHSS change guidance or procedures regarding the COVID-19 pandemic.

A drive through vaccination clinic will be held from 9 a.m to 4 p.m. every Saturday until September 18.

Note: This page will continue to be updated as additional information and guidance becomes available. Please also monitor your Triton email for forthcoming announcements.
Only some classes are being offered in online, hybrid, and/or hyflex formats. Check the Course Schedule or Web Advisor to see the format for each class. While you could register only for courses that are online, hybrid, or hyflex, there are likely other courses you need that will only be offered in-person. Please contact your academic advisor for assistance in planning your schedule.
Students attending only online classes do not need to submit a vaccination record UNLESS they plan to come to attend classes on-campus.
Students attending in-person classes for Fanuchånan 2021 will be required to have received their first COVID-19 vaccination
by Aug. 25, 2021.
Students taking in-person classes MUST be fully vaccinated or have an approved accommodation submitted to the Student Health Office using the secure online HIPAA-compliant form at https://url.uog.edu/submit-covid-info by Sept. 24, 2021.
Students registered for only classes that are in an online format are not required to be vaccinated.
NOTE: Because students taking in-person classes are required to be fully vaccinated or have
an approved accommodation, weekly negative COVID-19 test results will no longer be
accepted.
Students will need to provide a COVID-19 Vaccination Record Card (a copy or photo is acceptable) indicating full vaccination (one or two doses, depending on the vaccination brand received) that is stamped or signed by a CDC-enrolled COVID-19 vaccination provider.
Alternatively, students can request verification of their COVID-19 vaccination by making an appointment with the DPHSS Immunization Program. To book an appointment, call (671) 735-7143 or send an email to immunizationprogram@dphss.guam.gov. Requests should be submitted five to seven days in advance.
Those who were fully vaccinated at one of the enrolled COVID-19 provider clinics or pharmacies may also make a request for verification of their COVID-19 vaccination through their private health care provider.
FREE vaccination clinics are held at the UOG Calvo Field House and other locations around the island. You can view the schedule and make an appointment at: https://getimmunizedguam.org/covid-19-vaccination-schedule.
You will need to submit your documents by Sept. 24, 2021, via this secure, HIPAA-compliant form.
All in-person classes will be set up so that students and the instructor will be spaced six feet apart. Space is limited, so students are advised to register early.

All employees and students who will be on campus or involved in face-to-face classes, regardless of the amount of time, must be fully vaccinated or have an approved accommodation submitted using the secure, HIPAA-compliant form below. Copies or photos of your documents are acceptable.

Students or employees who are seeking an accommodation must submit evidence of their medical reason, disability, or their sincerely held religious belief, practice, or observance to the EEO/ADA/Title IX office.
