How To Submit Forms
Students have two options to submit their Financial Aid Forms:

We are open to the public from Monday through Friday from 8:00 am to 5:00 p.m. We will accept documents only between 8:00 a.m. and 4:30 p.m.

Forms for the 2026-2027 Academic Year (Fall 2026, Spring 2027, Summer 2027)
| Forms | Purpose |

| FAO Information Form | Collects student demographic information and provides for FERPA exceptions. Required to be submitted once every Academc Year. |
| Request for Certification - GI Bill | For students who are utilizing their GI BIll Education benefits. This form is required to certify your courses for the semester. |
| Consortium Agreement Form (GCC) | Form required for students who are concurrently enrolled at UOG and at Guam Community College |

| Total and Permanent Disability Discharge - Borrower Acknowledgement | Form required for students who were reported to have one or more Federal Direct Student Loans discharged due to a Total and Permanent Disability. |

| Maximum Credits/Time Limit Appeal Form | Form required to submit to request reinstatment of financial aid after meeting or exceeding a student's maximum allowed attempted credits in accordance with the Satisfactory Academic Progress Policy. |
| Dependency Override Request | Form to submit to request change in dependency status (Dependent to Independent status) |
| Request for Review of Financial Need Appeal Form | Form to submit to request revision of financial need status due to changes in income levels. |
