How To Submit Forms
For the 2023-24 Academic Year - Fanuchanan(Fall) 2023, Fanomnakan (Spring) 2024, and Tinalo (Summer) 2024 -, students have two options to submit their Financial Aid Forms
- Secure Upload **Preferred Method**
- Upload directly on your Financial Aid Self-Service account (How to upload documents in self-service)
- Submit at Financial Aid Office
- We have modified office hours during the pandemic emergency. We are open to the public from Monday through Thursday from 8 am to 4:30 p.m. and on Fridays from 8 a.m. to 2 p.m.

| FAO Information Form | Collects student demographic information and provides for FERPA exceptions. Required to be submitted once every Academc Year. |
| DEPENDENT Verification Worksheet (Group 1) | For Dependent students selected for verification under verification Group 1 |
| DEPENDENT Verification Worksheet (Group 4) | For Dependent students selected for verification under verification Group 4 |
| DEPENDENT Verification Worksheet (Group 5) | For Dependent students selected for verification under verification Group 5 |
| Dependent Non-Tax Filer Form | For dependent students whose parents and/or themselves do not intend or are not required to file a tax return. |
| INDEPENDENT Verification Worksheet (Group 1) | For Independent students selected for verification under verification Group 1 |
| INDEPENDENT Verification Worksheet (Group 4) | For Independent students selected for verification under verification Group 4 |
| INDEPENDENT Verification Worksheet (Group 5) | For Indeependent students selected for verification under verification Group 5 |
| Independent Non-Tax Filer Form | For independent students who do not intend or are not required to file a tax return. |
| Request for Certification - GI Bill | For students who are utilizing their GI BIll Education benefits. This form is required to certify your courses for the semester. |
| Instructions for Requesting Federal Student Loans | Detailed instructions for completing your Master Promissory Note and Entrance Counseling required to request federal student loans. |
| Loan Request Form | Form used to request federal student loans |
| Federal Loans Change Form | Form to request change in Direct Loan award amounts. |
| Request to Review Financial Need Status | Form to submit to request revision of financial need status due to changes in income levels. |
| Dependency Override Request | Form to submit to request change in dependency status (Dependent to Independent status) |
| Satisfactory Academic Progress Appeals Form | Form required to submit to request reinstatment of financial aid after failing to meet the reguirement of the Satisfactory Academic Progress Policy. |
| Consortium Agreement Form (GCC) | Form required for students who are concurrently enrolled at UOG and at Guam Community College |
